Collaboration Council
The senior decision-making table of the partnership. Leaders from every sector — hospital, primary care, community support, home care, long-term care, mental health — plus client and family advisors set SOHT’s strategic directions and approve its governance structure (the March 2026 structure is now being implemented).
Primary Care Network Governance Committee
Family physicians and nurse practitioners from comprehensive, specialty-focus, walk-in and team-based practice settings who organize Scarborough’s primary care sector in OHT planning and give it a voice in OHT decision-making. Clinical co-leads: Dr. Donna Vlahos and Dr. Rosemarie Lall.
Client and Family Advisory Committee (CFAC)
Patients, clients, caregivers and family members who co-design SOHT’s initiatives with implementation teams, review change ideas, take part in the strategic retreat and wrote the Patient Declaration of Values.
Operations Committee
Oversees the sector tables, standing committees and integrated implementation teams, and reports to the Collaboration Council.
Governance Committee
Makes sure SOHT has the governance structures to meet its changing priorities while aligning with Ontario Health: evaluating how well the structures advance integrated care and how the sectors are organized within them.
Finance Committee
Ensures funding provided to SOHT is effectively managed and available for priority initiatives, primary care engagement and community engagement.
Quality and Performance Committee
Quality improvement across SOHT: helping Alternate Level of Care patients transition safely out of hospital, community mental health and addictions services that keep people out of the Emergency Department, and cancer screening uptake.
Digital Health Committee
Digital tools that improve care: secure messaging between providers in a patient’s circle of care, online appointment booking, Ontario’s eServices Program and the Toronto Health and Social Services Directory.
Privacy Committee
Develops SOHT’s privacy program: governance framework, policies and procedures, the Data Sharing Agreement between signing members, risk oversight, and privacy training for shared systems.
Communications Committee
Communications and engagement across the partnership and with the community: this website, social media, the partner and community newsletters, and engagement at community events with the Community Engagement working group.
Recovery and Sustainability Committee
Integrated care pathways (diabetes), preventive cancer screening events, and seasonal surge planning for flu, COVID-19 and RSV in priority neighbourhoods.
