What the numbers say
Scarborough’s attributed population is almost 850,000 people. SOHT’s first priority population was frail seniors living with mental health or addictions issues and multiple chronic conditions; the 2025–2028 Strategic Plan widens the lens to primary care attachment, mental health, diabetes, heart failure and COPD, and safe transitions out of hospital. Data by neighbourhood guides where partners place clinics, where surge planning concentrates each fall, and where interprofessional primary care team funding is prioritized.
